President's Letter

Who is in control of your revenue cycle? Usually third parties and fiscal intermediaries determine the amount and time when an organization will be reimbursed. This limitation stresses the importance of an integrated contract management application as part of the billing process.

Terms, conditions and reimbursement relationships are often not known until payment is rendered. There is inherent value in understanding reimbursement terms for a majority of revenue sources. In order to do this, a revenue cycle management solution needs to be fully integrated with a contract management application. Stand-alone contracting applications are beneficial for analysis and profitability studies, but unless those terms dictate and drive how an organization will bill and collect, it remains separate.

For each patient type, contract management must have the ability to build in rules, fees, codes, forms and edits to drive the billing and collection process. Once supported, the system can perform contractual adjustments at billing which specifies the total due from each payor source and outlines the real-time, real-net value to the organization. Upon remittance, billers determine if the amount received matches anticipated terms or pursue any discrepancy.

The revenue cycle begins at preadmission/admitting and continues through the clinical process to billing and collection. What happens in the background of automated reimbursement rules in an integrated contract management suite creates a greater level of automation and targeted, accurate collections.

Now Supporting Multiple User Interfaces

HCS appreciates the fact that no two healthcare organizations are alike. This goes beyond not only the specific healthcare management structure, but also how organizations support and extend applications to their users. With this in mind, HCS offers multiple interface capabilities in accessing INTERACTANT and how the user is presented with specific applications.

Web-Based Applications - The entire suite of INTERACTANT Applications can be accessed by using Microsoft Internet Explorer version 6.0 and higher and a secure connection to the Internet. This newly developed and enhanced web-based user interface (UI) limits the information technology requirements of a traditional

client server model while also easing support of remote PCs at end user facilities. Within this model are hot links and additional navigation features which enhance the user’s ability to access a broad range of information and functionality.


Client-Server Applications - In response to the unique requirements of clinical end users, HCS has created a client-based UI. Increasing regulatory requirements present clinicians with growing challenges as charting moves from paper to an electronic medium. Due to issues such as high staff turnover and patient care responsibilities, clinical users requested end user experience that emulated paper charts, increased user buy-in, and simplified training.

The clinical UI provides more screen information with increased navigation and access across the entire electronic medical record.

The deployment of the interface is entirely the choice of the organization. Since the database and master files remain the same, organizations can choose both options simultaneously.

Hospice Care Reimbursement

In a continual effort to understand and accommodate the needs of our clients, HCS constantly expands and enhances INTERACTANT Applications to support the broad range of the services they provide. Throughout the years of providing information systems, this expansion has included areas such as senior living, outpatient services, behavioral healthcare, and most recently hospice care.

The INTERACTANT Contract Management Module is the engine behind how billing is performed by the system, so adding care and services is often as easy as establishing new rules in order to properly process accounts. Demographic and payor information is entered via the registration application.

Through acquisition, a client recently began providing hospice services in addition to other lines of business. Since hospice care lends itself very well to the existing services currently offered by HCS, the addition of hospice requirements was easily supported.

Applications affected by the addition of this line of business include registration, contract management, EDI, billing and collections.

Client organizations benefit from a single point of revenue cycle operation. Integrating back-office systems at the end of the month can be extremely difficult as well as excessively time consuming. Delays such as these can leave organizations unable to react to problem areas efficiently enough due to the lack of up-to-date information. Such issues highlight the benefits of a single system for revenue cycle operations where all accounts can be processed and collected from one system.

Solutions for Long-Term Acute Care Hospitals

Long-term acute care hospitals (LTCH) are specialty hospitals designed for extended stay patients with chronic conditions. Patients admitted to LTCHs after treatment in traditional acute care settings have no further need for intensive diagnosis procedures, but require resource-intensive care not provided in nursing homes or skilled nursing facilities. Because of this specialization, an LTCH provider faces a number of challenges which INTERACTANT addresses.

Due to the complicated average length of stay requirements and rules regarding patient referrals, an LTCH must efficiently track the number and type of referrals. This ensures they receive patients

requiring extensive and complex care. To that end, HCS clients take advantage of the extensive referral reporting capabilities available in the INTERACTANT Marketing/Contact Module. The ability to report on the statistics and the reasons why patients are accepted or denied by referral source provides an important tool in managing referrals.

Another challenge is changing CMS regulations and the complexity of accurately calculating expected reimbursement. Our LTCH clients take advantage of a number of LTCH specific capabilities such as fully automated tracking of interrupted stays and calculation of covered

patient days. The calculation of occurrence and value codes, and contractual allowances specific to LTCH providers are also performed by the system. Extensive DRG reporting is available which includes “what if” scenarios to provide our clients with tools to ensure accurate reimbursement. Lastly and perhaps most importantly, HCS can quickly adapt INTERACTANT in response to new CMS regulations affecting LTCH providers.
